South Mountain Family Psychiatry does not bill out of network services on behalf of the patient. The patient is responsible for all fees at self-pay rates at time of service. We will provide a suitable statement the patient can use to self-submit for out of network reimbursement.
Some plans require pre-authorization. Please contact the member service number on your insurance ID card to confirm your requirements.
* Including Supplement F&G plans if Medicare is Primary.
** 'Commercial' plans including Oscar & Oscar Small Group, UMR, UHC.
