Our screening and registration process for new patients:

  1. Follow the link below to complete and submit the screening questionnaire below describing your needs.  All information is kept confidential and is used for provider awareness.  A member of our staff will review your information and may contact you by reply email for additional information.   Note: We continue to experience a higher than usual demand. Our staff makes every effort to respond to new patient requests in a timely manner.  Please allow 2-3 business days for a response.  

  2. Staff will provide additional registration and consent forms instructions. Consent and Office Policies are sent from our HIPAA-compliant digital intake system to the address provided during the telephone interview.   

  3. South Mountain reserves the right to cancel and/or reschedule intake appointments if the pre-visit registration information is not completed.  Staff will provide additional information during the telephone interview.
