From the I-10, West on Elliot to S. 51st Street, then South to Cheyenne Street. In the Awhatukee Executive Plaza, on the Southeast corner of S 51st and Cheyenne. Directly South of the U.S Post Office. Park in any open un-covered spot.

We are found in the Wellness Center, Suite 250, in the main building. Enter the main building through the large brown double doors.

On arrival, proceed upstairs to Suite 250 – on your left as you reach the top of the stairs or as you exit the elevator.

Be sure to sign-in for your provider on the reception kiosk just inside the suite 250 entrance to let them know you are in the waiting area.
